Cremation
Guidline for funeral directors - the CR01 form adjacent, is the same as the nationally standardised form issued by the Ministry of Justice in 2009, with the addition of two non-statutory forms for your convenience towards the back, and our 'Prelim' on the last page.
Funeral Directors from away are welcome to use their local medical and crematorium forms, with due consideration to the differences in 'disposal' offered by different crematoria.
Burial
Burials may only be arranged through the cemetery/crematorium office. Please call 01253 735 429 to book an appointment, allowing a minimum of 48hours notice, thank you.
Applications for pre-purchasing graves for future use, and purchasing graves for the sole purpose of erecting a memorial, can be submitted using the 'Pre-Purchase of Graves' form which is available on the right hand side of this page. (See also 'Grave and Headstone Purchaing' in the menu on the left hand of this page)
Burial and Cremation paperwork must be recieved at the cemetery/crematorium office at Lytham Park, no later than 10am on the working day prior to the day of the funeral.
